Imipenem 500 mg, cilastatin 500 mg
Indications/Uses
Lower resp tract, UTI, intra-abdominal, gynecologic, bone & joint, skin & skin structure infections; bacterial septicemia, endocarditis, polymicrobic infections.
Dosage/Direction for Use
IV infusion Adult Mild infection 1-2 g daily in 4 divided doses. Moderate infection 500 mg tid-qid or 1 g tid. Severe, life-threatening 500 mg qid to 1 g tid-qid. Uncomplicated UTI 250 mg qid. Complicated UTI 500 mg qid. Max: 50 mg/kg/day or 4 g daily. Reduce dosage in patients w/ impaired renal function. Prophylaxis post-surgical infections Adult 1 g on induction of anesth & 1 g 3 hr later. High-risk surgery: 2 additional 500-mg doses at 8 & 16 hr post-induction. Ped patients ≥3 mth 15-25 mg/kg 6 hrly. Max: 2 g/day. ≤3 mth (≥1,500 g body wt): 4 wk-3 mth 25 mg/kg 6 hrly, 1-4 wk 25 mg/kg 8 hrly, <1 wk 25 mg/kg 12 hrly.
Contraindications
Hypersensitivity.
Special Precautions
Hypersensitivity to β-lactams. Antibiotic-associated colitis (overgrowth of nonsusceptible organisms). CNS disorders &/or compromised renal function. CrCl ≤5 mL/min/1.73 m2. Patients on hemodialysis. Pregnancy & lactation. Ped patients w/ CNS infections or <30 kg w/ impaired renal function.
Adverse Reactions
Thrombophlebitis, local pain, induration, erythema. Allergic reactions, GI disturbances, haematological disturbances, increase in liver enzymes, oliguria/anuria, polyuria, acute renal failure, elevation in serum creatinine & BUN, taste perversion, hearing loss. Myoclonic activity, psychic disturbances, confusional states or convulsion, paresthesia.
Drug Interactions
Ganciclovir, probenecid.
